HAZARDOUS WASTE

Machine Shop – Sullivan County, New Hampshire
Violation(s): Failure to conduct hazardous waste determinations, lack of a hazardous waste training program, and failing to maintain three years of manifests.
Penalty: $4,000

Medical Facility – Merrimack County, New Hampshire
Violation(s): Failure to notify New Hampshire Department of Environmental Services (NHDES) of hazardous waste generation, lack of weekly inspections, lack of labels on hazardous waste containers, and lack of a training program.
Penalty: $24,900

Biotechnology Company – Cheshire County, New Hampshire
Violation(s): Failure to conduct hazardous waste determinations, inappropriate hazardous waste storage and lack of labeling, failing to maintain three years of manifests, and lack of universal waste storage labels.
Penalty: $4,160

Manufacturer – Rockingham County, New Hampshire
Violation(s):  Lack of a hazardous waste training program, inappropriate hazardous waste storage, and lack of waste container labeling.
Penalty: $7,500

Plastic Fabrication Facility – Strafford County, New Hampshire
Violation(s): Disposal of hazardous waste through evaporation, lack of hazardous waste determinations, inadequate weekly inspection documentation, incomplete training program, failure to make arrangements with emergency response organizations, and lack of a contingency plan.
Penalty: $34,237

Equipment Manufacturer – Strafford County, New Hampshire
Violation(s): Inadequate weekly inspection documentation, incomplete training program, lack of or improper waste container labeling, failure to make arrangements with emergency response organizations, inappropriate satellite waste storage areas, incorrect waste manifest number, unmarked universal waste storage containers,  and lack of a contingency plan.
Penalty: $25,649

Church and HVAC Company – Hillsborough County, New Hampshire
Violation(s): Lack of notification, inspection, and training for major asbestos abatement project.
Penalty: $7,679

Asbestos Abatement Company – Cumberland County, Maine
Violation(s): Improper decontamination and lack of employee training.
Penalty: $4,000

LAND

Construction Company – Lincoln County, Maine
Violation(s): Inadequate erosion control, discharge to freshwater wetlands, and damage to vernal pools.
Penalty: $7,375

Gravel Pit Operator – Waldo County, Maine
Violation(s): Operating a pit greater than 10 acres without a permit.
Penalty: $5,000

Violation(s): Providing false discharge monitoring reports.
